Comforting Care


When the information collected with the MDS shows a problem, it triggers the second part of the RAI -- the Resident Assessment Protocol, or RAP. The RAP is a guide to help us make a more detailed and complete assessment in that area. With the information from that assessment, our care team can develop a care plan designed to correct problems and manage health issues specifically for that resident.

For example, let's say the MDS showed that a resident had a change in behavior recently and has become withdrawn and confused. This would be a reason to proceed with a RAP. The RAP would guide us to look at many different things, including the resident's medications. And, indeed, we might discover that the addition of a new medication had caused a drug interaction that has an effect on mood and thinking. Changing that medication could correct the problem. The RAP helps us streamline our assessments to detect problems early and take action to correct them.
