The care team typically includes the Medical Director, the Director of Nursing and nursing staff, the Consultant Pharmacist, and other specialists who are specially trained in caring for people who are elderly, frail, or have special needs. In the team meetings, we formulate care plans for them and review many areas, including their medications.

The Medical Director is a physician who is responsible for supervising medical care in the nursing home. The Director of Nursing oversees and coordinates the activities of the nursing staff. The consultant pharmacist conducts medication regimen reviews that are required by federal regulations. In addition, the consultant pharmacist assesses each resident's response to medication therapy and provides medication-related information to the facility.
